HOW TO APPLY
Our placement process is designed to be clear, respectful, and collaborative.
Step 1: Submit a Placement Inquiry
Start by completing our placement inquiry form or contacting us by phone or email.
Step 2: Provide Basic Information
We will request essential details to understand the person’s needs, including:
• daily living support needs, mobility, cognitive, or health considerations, preferred move-in timeframe, funding source or service authorization (if applicable), emergency contact and care team information
Step 3: Review and Compatibility Screening
We will review the information to determine fit for the home, availability, and compatibility with current residents.
Step 4: Next Steps & Tour (if appropriate)
If the home appears to be a good match, we will discuss next steps and schedule a tour while maintaining the privacy and respect of current residents.
Placement is based on availability, eligibility, and home compatibility.
